Nearly 70% of Suffolk County homes are on a cesspool or a septic system. Most owners use the two words interchangeably, and on Long Island "cesspool" tends to mean all of it. They are genuinely different objects, they fail differently, and the difference decides what a contractor finds when the covers come off.
A cesspool
A pit. Waste flows in from the house, solids settle at the bottom, and liquid soaks out through perforated sides or an open bottom into the surrounding sand and gravel.
There is no treatment stage anywhere in that description. A cesspool separates and drains. It does not remove nitrogen, and nitrogen is the thing Suffolk County is trying to keep out of its groundwater and its bays.
Built before 1973, it is probably block and brick, and the county has said those are mostly in the category of failures because of how they were built. They fail when the surrounding soil seals up with the biological mat that forms against it, or when the water table rises to meet the bottom of the pool. Either way the liquid stops leaving, and the interval between pump-outs starts shrinking.
A septic system
A tank plus a leaching structure. The tank holds the waste long enough for solids to settle and scum to rise, and the relatively clear liquid in between flows on to a leaching pool or field, where it soaks away.
Better than a cesspool, because the tank keeps solids out of the leaching structure and the leaching structure lasts much longer as a result. Still not treatment in the sense that matters here: a conventional septic system removes very little nitrogen. It is a well-organized way of putting the same problem into the ground more slowly.
Fails at the leaching end, usually, and slowly — slow fixtures throughout the house before anything appears in the yard.
An I/A system
Innovative/alternative onsite wastewater treatment. A small treatment plant in the yard. Depending on the technology it moves air through the wastewater, or recirculates it through a medium, so that biological processes convert nitrogen into a form that leaves harmlessly before the water is dispersed.
It removes far more nitrogen than a cesspool or a conventional tank. That is the entire point, and it is what the money is for.
What you would see on the property afterward: tank covers that have to stay accessible, a control panel with the electrical components and alarms, and compartments for air intake and filtered vents. The county warns of extensive site disruption during installation.
What it needs: annual professional maintenance for life, a modest electricity supply, and a pump-out roughly every third year — see what it costs to run.

Which do I have?
Often the only honest answer is that somebody has to look. Some useful hints: a single cover in the yard with no tank ahead of it suggests a cesspool; a rectangular cover followed by one or more round ones suggests a tank and leaching pools; a control panel on an outside wall means you already have an I/A system and a maintenance obligation with it.
If you genuinely do not know, that is not a problem for a grant application — the county rebuilt the form so that "I don't know" is an acceptable answer to some questions, precisely because so many owners bought the house decades after the system went in.
Why any of it matters
Both of the first two are eligible for replacement under the grants, and both replace with the third. If you are pumping once or twice a year, the county's own position is that a failure is on its way, and a documented failure goes to the front of the line.
